Access Your Health Information Online with MyRecord Patient Portal

  Now available through the MHealth app by Meditech. 


MyRecord patient portal is an online tool gives you the flexibility to access your health information and other resources on your time and between visits to your healthcare providers. If you have not yet enrolled in MyRecord, you will need to do that first. Please see the instructions for enrollment below.

Your MyRecord Patient Portal will reflect your visit history and information from all The Medical Center hospitals except for The Medical Center at Albany. Your Patient Portal may also include visit history and information from Med Center Health physician practices. The Medical Center at Albany and Med Center Health Surgical Specialists do not report to MyRecord patient portal. Please visit the websites of each of the entities listed to access the patient portal information.

Important: On 12/1/2016 and 4/1/2017, Med Center Health Physician Practices upgraded to the MyRecord Patient Portal. Your health record will include health information and visit history beginning from the date the physician practice began reporting to the portal. If you have a question about your MyRecord Patient Portal account, please call 270-745-1271.

With MyRecord, you can:

  • View test results.
  • Review health information from your doctor or hospital visit.
  • View your most current list of medications.
  • Use your smartphone to pull up your patient portal information.
  • View appointments. 

Before You Enroll
To complete the enrollment process, you will need:

  • name
  • date of birth
  • medical record number
  • social security number
  • the email address you provided when you registered at one of our Med Center Health facilities.

If you have all of these items, click the "Enroll" button next to the MyRecord logo above. If you did not provide your email address to you physician's office or at the hospital at registration, you will need to contact The Medical Center Release of Information Department to provide your email address and enroll. MyRecord is completely secure, so you can be confident that your private information is protected. Only you – or an authorized family member – can access your information through MyRecord.
