Because our children are our future,
Commonwealth Health Corporation made the commitment to meet the health and dental needs of
all children who need special assistance. This outreach to our little ones became a reality in June 1997 with the
development of Guardian Angel Childrens Services.
The goal of Guardian Angel is not to replace
existing healthcare services, but to "fill in the gaps" so that no child goes
with unmet health needs.
SERVICES PROVIDED:
- Referrals to other health care providers, medical and social
specialists and a variety of community agencies
- Diagnosis and treatment of acute illness, including medication.
- Financial assistance as needed for medical and dental expenses
(including hospitalization)
- Education about growth and development, maintaining health, or
dealing with medical conditions
Guardian Angel accepts referrals from any
source. Families, friends, schools, social services, or health care providers can inquire
about services on behalf of a child.
Patients are seen by appointment and there
is no fee. A staff member is on hand to assess and manage cases individually.
REQUIREMENTS:
All children under six years of age in
Allen, Logan, Simpson, and Warren counties are eligible. Each case is considered individually and
confidentially.
There is no charge to
the patients family for Guardian Angel Childrens Services. All services
are provided at Commonwealth Health Corporation's facilities or its designees.

The office is located at 740 East 10th Street at the Commonwealth Health
Free Clinic. Facilities are accessible to the handicapped.
