Registration
Pre-registration is required for all offerings to ensure adequacy of space and material.
Registration may be done by any of the following methods:
Cancellation & Refunds
For CHC employees: In the event of a cancellation of an offering, CHC employees will receive a message via
Microsoft Outlook®.
All CHC employees should review their e-mail and voice mail messages prior to attending an offering.
Cancellations and scheduling changes are also noted in the EDU Quarterly
calendar.
Employees and non-employees can also call the Education Registration and
Cancellation
phone line at (270) 745-1588 prior to attending class, to find out about class
cancellations or changes.
The Education Department reserves the right to cancel any offering or substitute an equally qualified speaker. Offerings having five (5) enrollees or
less may be canceled at the instructor’s discretion. This offering may be rescheduled at a later date.
During inclement weather, registrants may verify the cancellation of an offering by contacting
Education at (270) 796-2566.
All registrants who wish to cancel must notify Education at (270) 745-1588 within two (2) working days prior to the scheduled offering. No refunds will be issued after this deadline. There will be an additional $5.00 processing fee. Please allow 3-4 weeks to receive a refund.
In the event that an offering is cancelled, registration fees will be refunded in full.

Participant Fees
Participants who are employed by Commonwealth Health Corporation (CHC) will have complimentary admission to any program sponsored by CHC when an ID badge is presented. The cost of books and/or workbooks will be charged to the employee’s work unit. If an employee chooses to take a class not approved by the unit manager, and a book and/or workbook is a class requirement, the employee will be responsible for the cost of these materials. For non-employees, all fees include books and materials.

campus.

CEU Credits
The number of contact hours awarded for successful completion of each offering is listed
with the class information. Participants are required to have their
current license number and social security number when registering for
offerings. To receive contact hours participants must attend entire
offering and participate in any pre or post testing when applicable.
Partial credit will not be given.

Maintaining Continuing Education Records
The CHC Education Department maintains files on all CEU classes it has provided for five years. Employees are required to maintain a file of all CEU certificates earned. The Kentucky Board of Nursing
requires that each licensee retain the certificate of attendance in his or her
personal file for a minimum of twelve years, and the Kentucky Board of
Respiratory Care for a minimum of five years, for contact hours earned by
successful completion of offerings presented by Board-approved providers. All licensees from all departments should retain their attendance certificates in their personal files for a minimum of five
years.
